Nurse for a Day

RELEASE AND LIABILITY WAIVER, ASSUMPTION OF RISK, IDEMNIFICATION, CONSENT TO FIRST AID TREATMENT, CONSENT TO BE PHOTOGRAPHED, FILMED, OR VIDEOTAPED  

As the parent or legal guardian of a minor child/guardian (hereinafter child or student) I willingly and voluntarily agree that my child can participant in Marywood University's Nurse for a Day event to be held on March 5, 2026, and further, assume all risks of participation in this event. Participants will have the opportunity to engage in activities such as Stop the Bleed Training and simulation and nursing lab experiences. Participation is voluntary. At any time, my student can request to discontinue the activity.  The University will honor such request by immediately terminating participation. In the event my child suffers injury, I give my full permission for and consent to such first aid as deemed necessary to be provided to my child on Marywood’s campus or prior to transport to a hospital for further treatment. For myself and on behalf of my minor child, I do hereby discharge and release Marywood University, Marywood’s Nursing Department, Marywood’s Trustees, officers, employees, students and volunteers (hereinafter RELEASEES) from any and all liability, demands, claims and/or causes of action arising from or related to or attributable to any loss, damage, or injury, including death, that occurs as a result of the negligent and intentional acts or omissions of  my child participating in Nurse For The Day and/or my actions. I attest and verify that my child is medically able to participate in this event. For myself and on behalf of my minor child, I knowingly and freely ASSUME THE RISKS of our participation in this event. Further, I agree to hold harmless the RELEASEES responsible for any injury or loss, including death. I further agree to INDEMNIFY AND HOLD HARMLESS THE RELEASEES with respect to all injury, disability, death or loss to our persons or property, even if caused by my or my child’s negligent and intentional acts or omissions or the negligence or intended acts or omissions of the RELEASEES. For my child and myself I agree that this RELEASE, WAIVER OF LIABILITY, ASSUMPTION OF RISK, INDEMNIFICATION, and CONSENT TO TREATMENT, shall be governed and enforced by the laws of the Commonwealth of Pennsylvania with venue only in the Court of Common Pleas of Lackawanna County. I hereby authorize and give permission to Marywood, and those acting pursuant to its authority, to photograph, film, or videotape my child. I release Marywood and those acting on its behalf from any liability for any violation of any personal or proprietary right I or my child may have with respect to such use. All recordings and records shall remain Marywood’s property.
